Our friendly staff is always ready to help with scheduling, insurance questions and billing, or anything else you may need during your time with us. For your convenience, we have made the following information available.

Appointments:

To schedule an appointment with Boldt Dental, please call (541) 772-2756 or complete our online Request an Appointment form. If at any time you are unable to keep your appointment, please notify us 24 to 48 business hours in advance. We would be glad to reschedule your appointment at a more favorable time, and appreciate the advanced cancellation notice as we may be able to help another patient in your absence. We are thankful and mindful of your time and look forward to seeing you.

Insurance:

Boldt Dental accepts and bills to most major insurance companies. We are preferred providers for BlueCross/BlueShield, ODS, Delta Dental, and Advantage Dental insurance plans. Please contact your insurance provider to find more information about your dental plan.

Boldt Dental is an ODS Premier Provider, and a preferred provider for both MetLife, Lifemap, MODA, Aetna, United Concordia, Washington Dental, Advantage Dental, Pacific Source.
Billing and Financial Responsibility:

We accept most major credit cards. Please ask us about financing options including CreditCare. Please contact our payment specialists at 541-772-2756 if you have questions about payments or your billing statement.
